Remember to bring your child's shot record to every visit!

 

If you have insurance and it covers vaccines, please bring your insurance card to the clinic.  The clinic staff will be able to answer any further questions.  If you do not know if your insurance covers vaccines, call you insurance company and ask.  At this time, the childhood vaccines will still be offered free of charge to you.
